“Immunogenetics” Real-time PCR based SNP genotyping assay allows detection of a range of genetic polymorphisms associated with immune mediated disorders or immune system dysregulation. The employed Real-time PCR method discriminates alleles thus providing genotype information that helps to estimate odds of disease development or to evaluate functional properties of key biomolecules involved in regulation of immune response.

Now it is well established that changes in cytokine gene cluster (IL28A, IL28B and IL29), localized on chromosome 19 (19q13) of a person, are the main determinant of anti-viral defense (Fig. 39). Polymorphism in the region adjacent to the interleukin 28B (IL28B) is of greatest importance. Both of these polymorphisms label the portion that is important for response to antiviral therapy.

Identification of a patient’s genotype on IL28B may change the treatment algorithm (change in the duration of standard therapy PEG-IFN/RBV or triple CHC therapy). Optimization of therapy will help avoid side effects and further additional costs for triple therapy with protease inhibitors (telaprevir and boceprevir).

IL28B REAL-TIME PCR genotyping Kit is intended for detection and allelic discrimination of SNP’s, associated with IL28B function. The results of this genetic test can be used for evaluation of hepatitis C antiviral therapy efficiency (OMIM #609532).

Indications for genetic analysis:

• Prediction of disease outcome;

• Choice between standard and pegylated interferon plus ribavirin combination therapy for CHC treatment;

• Choice of the tactics “waiting for accessibility of triple therapy with HCV protease inhibitors” or treatment according to the standard scheme of double interferon plus ribavirin combination therapy for patients with additional factors (except liver fibrosis) that reduce the likelihood of treatment with drugs of existing standard antiviral therapy.
